Nest Medication Care provides Home Medicines Reviews (HMRs) and medication-management services for general practices. 

Refer for an HMR

GPs can forward an HMR/DMMR referral or relevant consultation letter directly to Nest Medication Care at nestmedicationcare@gmail.com 

Nest will contact the patient, arrange the review by a credentialled pharmacist and return a comprehensive medication review report to the referring GP.

Nest can also work directly with your practice to provide:
